Brantford-Brant is one of 13 new Ontario Health Teams approved this week by the province in a bid streamline patient care between home settings, hospitals and long-term care.
Members from the 14 local organizations have been working for 1 1/2 years to get to this point, said Sherry Kerr, co-chair of the local team with Dr. Scott Elliott.“It’s been a long road,” said Kerr, who is also executive director of Participation Support Service, formerly Participation House Brantford.
“We were actually hoping for approval last year at this time but COVID has really slowed things. We were told in the summer to get our final documents ready for approval so we were pretty excited by that.”
The local team comprises the Brant Community Healthcare System, John Noble Home, St. Joseph’s Lifecare Centre, Grand River Community Health Centre, Canadian Mental Health Association, Participation Support Services, Community Paramedics. Primacare Community Family Health Team. Brantford Brant Primary Care Council, Family Counselling Centre of Brant, Woodview, St. Leonard’s Community Services, Adult Recreation Therapy, the Aboriginal Health Centre and the Alzheimer’s Society.
